FREE NHS HEALTH  CHECK
Ages 40 - 74 years?


You may be entitled to a fee NHS health check.  This health check can help to detect diabetes, high blood pressure and high  cholesterol . The check will also assess your risk of developing cardiovascular disease in the future and with the right measures can prevent these diseases.

 

To qualify for your health check you must be aged between 40 - 74 years and NOT ALREADY receiving treatment for: Heart disease, Kidney disease, stroke or diabetes

 

To arrange your free health check please contact the surgery on 01257 791216 and ask for an appointment for your diabetes and cholesterol blood test followed by an appointment with our health care assistant

New Patients


New patients may register with the practice if they live within our boundary area


 


Patients previously registered with the Practice


Patients previously registered with the practice may re-join providing they live within our boundary area


 


Patients registered with the Practice who are moving house


If you are moving outside our boundary area you will have to register with a new Doctor within 2 weeks of moving


 


If you lived outside our area when the boundary was established some years ago you will have been able to remain at the practice. However, if you move from that house to another address that is currently outside our boundary we will ask you to register with another Doctor
