FREE NHS HEALTH  CHECK
Ages 40 - 74 years?


You may be entitled to a fee NHS health check.  This health check can help to detect diabetes, high blood pressure and high  cholesterol . The check will also assess your risk of developing cardiovascular disease in the future and with the right measures can prevent these diseases.

 

To qualify for your health check you must be aged between 40 - 74 years and NOT ALREADY receiving treatment for: Heart disease, Kidney disease, stroke or diabetes

 

To arrange your free health check please contact the surgery on 01257 791216 and ask for an appointment for your diabetes and cholesterol blood test followed by an appointment with our health care assistant

 

 

Test Results

test_sampleThe practice will always contact patients where they have had a test and further action is required, patients are advised to ensure we have their up to date mobile telephone number as we can text you should any further action be needed. If you do not have a mobile telephone please ensure we have your up to date daytime telephone number.

If you would like to enquire regarding your test results you can call 01257 442524 from 9am - 3pm Monday to Friday.

The practice has a strict policy regarding confidentiality and data protection and we will only release test results to the person to whom they relate unless that person has given prior permission for the release of this data.

We will always contact you if your test result requires any further action. Please ensure we have your up to date contact details in case we need to contact you urgently.
