FREE NHS HEALTH  CHECK
Ages 40 - 74 years?


You may be entitled to a fee NHS health check.  This health check can help to detect diabetes, high blood pressure and high  cholesterol . The check will also assess your risk of developing cardiovascular disease in the future and with the right measures can prevent these diseases.

 

To qualify for your health check you must be aged between 40 - 74 years and NOT ALREADY receiving treatment for: Heart disease, Kidney disease, stroke or diabetes

 

To arrange your free health check please contact the surgery on 01257 791216 and ask for an appointment for your diabetes and cholesterol blood test followed by an appointment with our health care assistant

 

 

CANELLING YOUR APPOINTMENT

Please cancel your appointment in good time to allow us to use it for another patient. even if it is short notice, it is better to cancel your appointment rather than failing to attend

You can cancel your appointment by:

  • Text message - If you received an appointment reminder by text, you can cancel by following the instructions on the text message.
  • Telephone reception:  01257 442524
  • Cancel online via your online account 
  • Cancel via the NHS App

If you have a mobile telephone we can text you an appointment reminder to ensure you do not forget your appointment. Please ensure we have your up to date mobile telephone number.

The practice monitors repeated failures to attend appointments,  action will be taken if patients repeatedly fail to attend appointments.
